The Complex Care Coordination Team (CoNeCT) provides a comprehensive care coordination service to patients in the community with the aim to reduce avoidable hospital presentations.Currently we are seeking a dynamic and motivated Team Leader who shares our organisation values, to lead and manage the metrowide CoNeCT service. This is additional FTE that has been established to provide increased managerial support within the service.Through community-based case management and care coordination, CoNeCT helps individuals at high risk of poor health outcomes due to complex medical and social needs. These individuals typically have difficulty navigating health and social care systems, often becoming high intensity users of the system at risk of avoidable Emergency Department (ED) presentations and lengthy unplanned inpatient stays. CoNeCT has demonstrated consistent positive outcomes for individuals engaged with the service, as well as the system by reducing pressure on EDs and inpatient beds.The CoNeCT Team Leader is responsible for operational oversight of all aspects of CoNeCT service delivery, achieving program targets, continuous development of the service, and for providing a specialised level of clinical consultancy service within the service.CoNeCT is a metropolitan wide service with bases in Nedlands, Perth, South Guildford, Armadale, Myaree and Rockingham.South Metropolitan Health Service is where jobs become careers, passion finds its purpose, and your voice finds its place.
