The Complex Care Coordination Team (CoNeCT) provides a comprehensive care coordination service to patients in the community with the aim to reduce avoidable hospital presentations.Currently we are seeking a dynamic and motivated Team Leader who shares our organisation values, to lead and manage the metrowide CoNeCT service. This is additional FTE that has been established to provide increased managerial support within the service.Through community-based case management and care coordination, CoNeCT helps individuals at high risk of poor health outcomes due to complex medical and social needs. These individuals typically have difficulty navigating health and social care systems, often becoming high intensity users of the system at risk of avoidable Emergency Department (ED) presentations and lengthy unplanned inpatient stays.
