About the Service InSync Youth Services provide support and accommodation to young people experiencing or at risk of homelessness.
Our programs include: Crisis accommodation for young people 16 to 18 years of age.
On site staff provide case planned support to assist young people to gain living skills, build resilience and independence.
Transitional accommodation for young people, 16 to 25 years of age and young families.
Offsite staff provide case planned support to assist young people to gain living skills, build resilience and independence.
Centre based/mobile support in the Beenleigh and the Redlands areas.
Staff provide a broad range of support, information, and referrals for young people from high school age up to 25.
About the role The overall purpose of the Residential Practitioner is the provision of strength-based case management support to young people experiencing homelessness who enter our residential settings either in crisis accommodation or transitional community-based housing.
Residential Practitioners support Youth Workers in the development of support plans and will also provide periods of post accommodation follow up and support to young people exiting our accommodation services to ensure their ongoing wellbeing and accommodation stability.
Key to success in this role will be strong, constructive relationships with young people, professional colleagues and external agencies including the development of effective referral relationships and service networks.
We 2 vacancies available for 12 Month Fixed-Term Contracts, with possible extensions - working 60 hours per fortnight based at our Mount Gravatt East services.
Roster includes 7 Shifts per fortnight working 2:00pm to 10:00pm, evening work including every 2nd weekend.
Responsibilities: Provide trauma-informed case management to support young people in short-term and transitional accommodation, helping them set goals and secure long-term housing.
Assist clients in navigating and accessing housing, health, social security, and other support systems, and connect them with additional services as needed.
Build and maintain relationships with housing providers and represent the service at relevant meetings.
Maintain accurate client records and meet data collection and reporting requirements.
Manage client and staff feedback surveys and complete additional administrative tasks as needed.
Follow and promote the service framework focused on client strengths, self-determination, and social justice.
Support quality practice enhancements and implement efficient, evidence-based methodologies.
Stay informed on current practices through relevant networks and conduct risk assessments to develop mitigation strategies.
Foster a positive team environment and actively contribute to continuous improvement.
Qualifications, Credentials and Other Requirements Current National Police Check Current Blue card (Working with Children) Current Queensland Driver Licence Degree in Social Work, Human Services, or a related field, and/or equivalent practical experience in the public and community housing sector Ability and willingness to travel within the service region.
Ability and willingness to work outside of standard business hours and provide on-call support as required.

Knowledgeable about housing services and support systems for at-risk youth.
Experienced in solution-focused, trauma-informed case management.
Familiar with team-based support planning and solutions-focused thinking.
Skilled in sharing knowledge with colleagues.
Excellent at building relationships and developing referral networks.
Effective communicator with strong negotiation skills.
