The purpose of the position is to create awareness of services offered amongst key stakeholders in the area, to identify patients aligned with admission criteria and to coordinate and provide a professional and efficient Case and Risk Management function.CRITICAL OUTPUTSParticipate in recruiting referring Doctors and other members of the professional team.Participate in recruiting referring Doctors, other members of the professional team and active marketing of the facility.Build productive relationships with healthcare professionals to ensure adequate referral rates to achieve budgeted occupancy levels.Conduct clinical assessments to ensure that transfers and admissions adhere to criteria.Obtain authorization from funders/medical aids & submit required documentation during the patient stay to ensure timely payment of accounts.Minimize the financial risk by effectively managing patients stay.Coordinate feedback from the multi-disciplinary team and communication on multiple platforms.Provide management, audit, progress, and discharge reports.Effectively manage and maintain patient outcomes data.Ensure compliance with case management procedures, contracts, and legal aspects by utilizing available resources and systems.REQUIREMENTSRegistered with SANC as a Registered NurseAt least 3 years' Case Management experience in a Sub-Acute or Acute hospital environmentPrevious experience in marketing services in a healthcare environmentWorking knowledge of ICD & CPT coding would be advantageousWorking knowledge of Fund's rules, limits & contracts; per diem/fixed fee interpretation; tariffs an advantageManage daily workflow, case management, and coding processesFull computer literacy on all MS Office packagesB2B EMS (electronic messaging systems: check status of authorization and used for problem-solving.Ability to build relationships with Health Care ProfessionalsExcellent Administrative & Negotiation skills requiredPatient admin programs on the AS400 systemIf your personal purpose is aligned with our purpose and your values fit in with our value system, please apply.
