Job Title: Licensed Social Worker Location: Nursing Home in New Orleans, LA Job Type: Full-Time About Us: Our nursing home client in the heart of New Orleans is dedicated to providing exceptional care and support to our elderly residents.
We are currently seeking a compassionate and skilled Licensed Social Worker to join our multidisciplinary team.
This role is critical in enhancing the quality of life of our residents through comprehensive social services.


Key Responsibilities: Conduct initial assessments of new residents to understand their social, emotional, and psychological needs. Develop, implement, and review individualized care plans in collaboration with the healthcare team. Provide ongoing support and counseling to residents and their families, addressing concerns related to adjustment, mental health, and end-of-life planning. Act as a liaison between residents, families, and the healthcare team, ensuring effective communication and advocacy for resident needs. Coordinate with external agencies for additional services such as home care, legal assistance, and financial support. Organize and facilitate group activities and programs aimed at promoting social interaction, mental stimulation, and emotional well-being. Maintain detailed records and documentation in compliance with legal and ethical standards. Participate in interdisciplinary team meetings to discuss care plans and resident progress. Provide crisis intervention as needed and make referrals to appropriate community resources. Conduct regular follow-ups to assess the effectiveness of interventions and adjust plans as necessary. Qualifications: Must hold a current license as a Social Worker in Louisiana. Master's degree in Social Work from an accredited institution. Experience working in a healthcare setting, particularly in geriatrics or a nursing home environment, is preferred. Strong understanding of the social, emotional, and psychological challenges faced by elderly individuals. Excellent communication, interpersonal, and counseling skills. Ability to work collaboratively in a multidisciplinary team. Knowledge of community resources and services for the elderly. Proficient in maintaining accurate and detailed documentation.
