Return To Country Care Coordinator

Our First Nations team based in Alice Springs are currently sourcing a Return to Country - Care Coordinator to join their team on a full-time basis.
This exciting role will see you working with our indigenous clients and managing the in-home aged care packages specific to the Troopy Program funding scheme. You will assess our client's needs, prepare care plans and work with the team to grow the knowledge of the program across our region and promote the use of our Troopy's to support elderly clients maintaining contact with their homelands.
This role would ideally suit a self-starter who is able to work independently with minimal supervision. You will hold the qualification (minimum) of Certificate III in Aged, Disability or Individual care and ideally be experienced working with First Nation clients within Central Australia.
You will reside in or be willing to relocate to Alice Springs. Relocation support of up to $5K is provided. In addition, this role may require you to support clients and travel with them to community. Therefore, you must be able to be flexible and travel when required with often overnight stays.

Skills & experience Qualified Enrolled Nurse with an unrestricted AHPRA registration.Current First Aid Certificate.Previous work in a similar role of Case Management or Coordination within Community Services, such as Aged Care, Disability, Mental Health, Housing, or a similar industry.Previous experience working with clients of dementia preferred however, not essential.Knowledge of culturally sensitive practice when working with Aboriginal and Torres Strait Islander people and people from culturally and linguistically diverse backgrounds, with preference to clients from Central Australia.Prior use of Microsoft Office suite and other online client reporting systems.Strong driving skills and ability to tow a trailer when required across long distances.Due to the manual requirements in this role, it is recommended applicants have strong physical fitness and ability to work in NT climate.Ability to travel regularly as part of the role to remote NT Communities which include overnight stays in remote communities.Before applying, you need to have the following A current resume demonstrating the required experience and 2 x references.A National Police Check dated or willingness to obtain a National Police Check to be dated within 3 months of commencement.A NDIS Screening check or proof of application.Our staff and volunteers are compassionate and knowledgeable members of the local community, who are committed to providing personalised care and support.
