mothers2mothers (m2m) is an African Primary Health Care organisation that employs women living with HIV as community health workers across 10 African nations. These "Mentor Mothers" deliver integrated health services at clinics and in communities to ensure everyone, everywhere, has access to the services they need to be healthy and stay in care. The Registered Nurse will play a critical role in supporting the implementation of these integrated health services at the Innovation Hub in Ikhwezi. This includes delivering an enhanced package of services aligned with m2m's strategic focus on addressing key health challenges. The primary focus of this role will be health promotion, disease prevention, case finding, screening, and linking individuals to treatment, care, and support for Tuberculosis (TB), Non-Communicable Diseases (NCDs), and Cervical Cancer. The successful candidate will contribute significantly to the effectiveness of m2m's health interventions, ensuring high-quality care and support for those in need.Key Performance Areas:Health education and promotion:Conduct and supervise health promotion activities focused on disease prevention and healthy lifestyle education.Promote awareness and preventive measures for HIV, TB, HPV, cervical cancer, hypertension, and diabetes and provide technical support and mentorship to peers in alignment with DoH guidelines and protocols.In collaboration with DoH and m2m Peer Mentors, lead community outreach campaigns focusing on engaging the community and ensuring access to preventive services such as HPV vaccination, TB screening, case finding, and NCD screenings.Primary prevention and screening:Conduct routine screenings for HPV, cervical cancer, hypertension, and diabetes for clients accessing services at the health facility.Collaborate/Link with school health services to ensure comprehensive HPV vaccination coverage for girls aged 9-12 years.Support the TB programme by supporting peers to conduct TB case finding, sputum collection, and contact tracing for individuals with confirmed TB or on TB treatment.Perform Pap smears for women referred after pre-screening and refer clients with pre-cancerous lesions for treatment.Provide guidance, mentorship and quality assurance to Peer Mentors in activities related to TB case finding, sputum collection, NCD screening.Referrals and Linkages:Establish and maintain effective referral pathways within the health facility for seamless linkage between services and community.Ensure timely referral of clients with pre-cancerous lesions, non-communicable diseases, or TB to relevant service points within the health facility and provide follow-up to ensure their continued care.Ensure clients adhere to treatment plans and monitor follow-ups for those in care for TB, NCDs, and cervical cancer prevention.Support peers ensuring that all referred clients are linked to care and have accessed required services.Relationships management:Maintain close collaboration with Department of Health clinical staff at the health facility to ensure cohesive service delivery.Foster strong working relationships with health facility teams through regular participation in health facility meetings and support outreach campaigns.Maintain relationships with the community-based partners and school health teams.Administration:Compile and submit weekly/monthly report.Timely submission of HR and finance documents.Participate in weekly team meetings, facility meetings and partner meetings.Qualifications and Experience:A minimum qualification of a BCur Nursing degree or equivalent nursing qualification as a Registered Nurse.Active registration with the South African Nursing Council (SANC).Primary Health Care Training / APC (Adult Primary Care), NIMART training will be an added advantage.5-10 years of experience in providing comprehensive Primary Health Care services, including health education, promotion, prevention, and management of non-communicable diseases, TB, cervical cancer, HPV, and immunization.Proficient in conducting Pap smears.Strong passion for community health work.Excellent communication and writing skills.Demonstrated ability to problem-solve, make decisions, and work independently with minimal supervision.Ability to manage multiple tasks, work under pressure, and meet tight deadlines.Exceptional communication and interpersonal skills, with a strong sense of cultural sensitivity.Highly organized, dependable, and detail-oriented.Proven ability to manage multiple tasks effectively while adhering to strict deadlines.Ability to work both independently and collaboratively within a team environment.Strong capability to perform well under pressure, meet deadlines, and handle follow-up tasks with minimal supervision.Proficient in Microsoft Office Suite and other relevant software.Excellent verbal and written English skills are essential.mothers2mothers is an equal opportunity employer.
