Mission Australia's Psychosocial Services in the Northern Territory help individuals with mental illness transition to the NDIS or provide support if they're ineligible This unique Case Manager opportunity will see you connect with people living in identified remote communities who are living with complex mental illness. Case management is provided in person and via tele-health so you will need to have the flexibility to travel regularly to Pupunya, Haast Bluff and Mount Liebig. What's in it for you?

What's the role?
Review referrals and evaluate eligibility
Engage with participants in a culturally appropriate manner, fostering connection and trust
Develop strong relationships with key stakeholders
Provide effective and recovery based approach case management
Create and maintain up to date individualised case management files and recovery plans
Complete a range of internal and external reports
Attend local community meetings, forums and community awareness events and conferences as required
Is this the right role for you?
Minimum Certificate IV qualifications in Mental Health or equivalent with previous experience working in the mental health sector
Strong understanding of Aboriginal and Torres Strait Islander cultures and communities – previous experience working within remote or regional groups
Ability to travel to remote communities on a regular basis, involving overnight stays
A sound understanding of the National Disability Insurance Scheme (NDIS) access process
NT PHN/RWA NT may be able to offer relocation support and clinical educational financial assistance to health professionals who are relocating to the NT or moving within the NT to a greater remoteness area classification for the purposes of employment and further clinical education.
