We are seeking a friendly and compassionate Primary Care Provider to join our team. As a Primary Care Provider you will play a crucial role in providing comprehensive care to patients in long-term care facilities. Responsibilities: Provide comprehensive healthcare services and coordinate patient careWork closely with a team of healthcare professionalsAssess, diagnose, and treat residents' physical and mental well-beingConduct thorough patient assessments, including medical history reviews and physical examinationsOrder and interpret diagnostic testsDevelop accurate diagnoses and treatment plansPrescribe medications and administer treatmentsProvide counseling to residents and their familiesCollaborate with the interdisciplinary team for seamless coordination of careManage chronic conditions and monitor progressMake appropriate adjustments to care plansCommitment to person-centered careCreate a safe and nurturing environment for elderly residents in the nursing home.Requirements: Nurse Practitioner or Physician Assistant LicenseDEA License6 months experience in long-term careSalary Range: $150,000.00 - $200,000.00
