You will work with individuals in their own home, leading on non-complex assessment on discharge from hospital and delivering targeted and intensive rehabilitation programmes to maximise patient independence.You will work alongside our existing Band 4 practitioner within Home First as well as Band 3 care support workers dedicated to the service.This is an opportunity to work within an established Integrated team in a community-based setting focusing on facilitated discharge and prevention of admission.You will be expected to manage a caseload to provide assessment, rehabilitation, onward referral, and discharge to a range of patients referred to the service. This reduction will also be applied pro-rata for part-time staff. This advert and any subsequent offer/contract of employment therefore reflects the new working hours. However, as not all service areas will be able to adopt the 37-hour working week immediately from 1 April 2024, you may be required to work up to an additional 30 minutes per week for a temporary period for which you would be paid until the service you are working in changes rosters or working patterns to accommodate the new reduced working week.
