MINIMUM REQUIREMENT:Grade12A Nursing Diploma or a degree in Nursing is essential Current registration with the South African Nursing Council (SANC)Certificates in Nurse-initiated Management of ART (NIMART) and Adult Primary Care (APC) are essentialA dispensing license would be an advantageValid South African Nursing Council Practice CertificateDrivers license essential as this is a community-based role EXPERIENCE:Five years' experience in HIV prevention, treatment, care and support programmes and TB service provisionKnowledge of HIV management, including the UNAIDS 95-95-95 goals and targets, and of TB managementKnowledge of Ideal Clinic Realisation and Management (ICRM) and QIP frameworksClinical care: understanding of the Universal Test and Treat (UTT) approach, QI processes and concepts, and competence in HIV clinical management (NIMART)Experience in provider-initiated HIV testing and counselling (PITC), including index case contact testing, is essentialExperience in facilitation of HIV self-screening would be an advantageGood understanding of the South African public health system, including national and provincial policies and management structuresExperience in providing supportive supervision and mentoring to junior staff working at the same facilityPrevious work experience in NGOs or funded programmes will be an advantageComputer literacy and sound knowledge of Microsoft Office (Excel, Word, PowerPoint, Outlook)KEY RESPONSIBILITIES:Provide services using evidence-based HIV and TB interventions in all streams according to the National and Provincial Department of Health (DoH) policy, guidelines and standard operational proceduresProvide HIV Testing Services (HTS) at communities in identified target areas, and initiate patients on antiretroviral therapy (ART)Implement the Universal Test and Treat (UTT) policy according to the latest guidelines to support the UNAIDS 95-95-95 targetsManage patients according to their viral load results – transitioning to DMoC, retaking of blood samples, or referring them for enhanced adherence counselling (EAC) or for secondline treatment regimensPromote differentiated care options for stable patients by creating demand for the CCMDD serviceIdentify and enrol eligible patients for transition to TLDScale up CCMDD patient registration on the electronic SyNCH systemIdentify patients who are due for viral load tests at six and 12 months, according to the project guidelinesMonitor viral load tests done on a weekly and monthly basisSupport implementation of huddle meetingsMentor and Coach other clinicians on quality clinical care of patientsIdentify and document good facility practices from implemented Quality Improvement Plan (QIP) activitiesActively transfer and check the successful transfer of patients to other clinicsParticipate in the facility's weekly and monthly Operation Phuthuma huddle meetingsParticipate in data analysis (facility targets, tracer indicators, cascades)Submit daily, weekly and monthly data internally and to the facilityShould you not receive a response within 2 weeks, kindly consider your application unsuccessfulCLOSING DATE: 19 June 2024
