Position Summary:This position is responsible for managing system-wide Social Driver of Health screening, referral, and intervention programming for UMass Memorial Health (UMMH) patients and value-based program members, along with other population health and value-based program efforts that require partnerships with community and government partners. Works collaboratively with community benefits offices, social work, clinical practices and departments, and other internal and external stakeholders to implement meaningful community resource referrals and social driver of health intervention programs for patients at UMMH. Ensures compliance with regulatory and payer-requirements around social health and related population health initiatives. Major Responsibilities:Leads development and maintenance of a system-wide Social Drivers of Health (SDOH) navigation system for UMMH patients by partnering with broad set of internal and external stakeholders, including but not limited to Community Benefits Offices, Social Work, Case Management, clinical practices and departments, provider associations, government partners, and local community service organizations.Builds and maintains ongoing, bi-directional relationships with community service organizations that can receive referrals for SDOH follow-up and support; leverages and coordinates with community benefits offices across the system to ensure coordinated support to community service organizations.Serves as the point person for the relationship with contracted vendors supporting SDOH screening and navigation (e.g. findhelp and Get Well). Monitors deliverables, coordinates contracting process for new agreements and updates, troubleshoots problems, and completes all due diligence with legal, IT, and relevant departments prior to senior leadership review and approval.In partnership with team, ensures effective implementation of SDOH screening in multiple settings across the health system, and ensures appropriate integration of screening with follow-up community resource navigation. Supports updates to screening workflows as needed to ensure compliance with SDOH regulatory requirements. Participates in and leads planning and implementation of training for end users. Manages team and provides coordination and meaningful supports—including training, connection-building, and barrier reduction—as needed across the UMMH system. Collects feedback and ideas and coordinates implementation of solutions to improve efficiency and success of community resource navigation programs.Liaises with OCI and Tufts Health Plan leads for the Flexible Services program and other SDOH programming included in value-based programs. Position Qualifications: License/Certification/Education: Required:Bachelor's degree.Preferred:Doctoral or Master's degree in public health, public policy, social work, or a related clinical or administrative area within healthcare or public health.Experience/Skills:Required:Minimum five (5) years of related experience, including three (3) years of leadership experience.System level thinking in assessing patterns, identifying problems or opportunities. Excellent organization and program management skills, including written and verbal and presentation skills to facilitate workgroups, prepare materials, report summaries, and general communications.Demonstrated experience collaborating with and coordinating an array of people and data initiatives across the organization and with external agencies.Ability to lead and function within a diversified team.Demonstrates ability to work independently; be self-directed to complete the goals and objectives of the position.Excellent interpersonal skills, including the ability to establish and maintain effective working relationships with physicians, administration, and staff.Capable of managing multiple projects simultaneously, and of operating in a highly matrixed organization where authority and responsibility for planning and execution are often shared across departments and individuals.Preferred:Knowledge of and/or existing relationships with community organizations in Central Massachusetts.Proven history of engaging communities that have been socially and economically marginalized.Experience working within hospital/healthcare delivery systems.Experience working with community health workers and/or social workers.All qualified applicants will receive consideration for employment without regard to race, color, religion, sex, sexual orientation, gender identity, national origin, disability or protected veteran status.We're striving to make respect a part of everything we do at UMass Memorial Health – for our patients, our community and each other.
