We are more than 16,000 employees, working together as one health system in a relentless pursuit of healing for our patients, community and each other.Major Responsibilities:Assess the physical and psychosocial status of patients in all settings by obtaining appropriate patient histories, administering physician/primary care provider orders, nursing assessment, medication administration reconciliation, taking and monitoring vitals of clients on the unit, medication administration and documentation.Renders and supervises care of clients on the unit.Monitors medication and consultation with prescribers.Provide appropriate health care and coordinate referrals as prescribed by providers.Participates in the evaluation of client's condition and progress with team members.Promote education and adherence to prescribed treatment protocols.Facilitates wellness and medically related psychoeducational groups to individuals.Records all client information within electronic medical record system daily.Assists RN on daily basis.Offers nursing consult on emergency service evaluations.Works with clients to coordinate care with their PCP, psychiatrist, and other professionals including but not limited to medication refills and follow-up medical appointments.Standard Staffing Level Responsibilities:Complies with established departmental policies, procedures, and objectives.Attends variety of meetings, conferences, seminars as required or directed.Demonstrates use of Quality Improvement in daily operations.Complies with all health and safety regulations and requirements.Respects diverse views and approaches, demonstrating Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients, and visitors.Maintains regular, reliable, and predictable attendance.Performs other similar and related duties as required or directed.All responsibilities are essential job functions.Position Qualifications:License/Certification/Education:Required: Graduate of an accredited school of Nursing with current license as a Licensed Practical Nurse in the state of Massachusetts.Satisfactory CORI background check.Driving is not a requirement.Preferred: N/AExperience/Skills:Required: Demonstrates proficiency in completing electronic documentation in compliance with grant requirements, agency, state, and federal guidelines and policies.Demonstrates ability to communicate effectively and professionally with clients, staff, and providers.Preferred:One (1) year of nursing experience.Knowledge of medical, substance abuse, and mental health nursing.Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.Equal Opportunity EmployerAll qualified applicants will receive consideration for employment without regard to race, color, religion, sex, sexual orientation, gender identity, national origin, disability or protected veteran status.We're striving to make respect a part of everything we do at UMass Memorial Health – for our patients, our community and each other.
