Organisational Background
Centre for Community Impact (CCI) is a non-profit South African organisation based in Pretoria. We
aim to be a center of excellence in strategic health and development communication programs that
are centered around meaningful community engagement.
Position Summary
The Linkage officer will be responsible for the successful navigation and linkage of Adolescents and
Young People (AYP) and Male Sexual Partners (MSP) to various services in the Adolescent and Young
People's Programme. They will support same day initiation linkage of clients to ART, TB and STI
treatment and PrEP support and adherence. They will also provide linkages to psycho-social and
structural support services. The incumbent will work closely with key stakeholder to ensure a
consistent uptake of Linkage to care and support in the programme.
Primary Responsibilities
• To support linkage to care of clients enrolled in the AYP programme.
• Facilitate initiation on ART of HIV infected clients.
• Support professional nurses to trace partners of positive index clients if permission was granted
by the index client.
• Provide HTS services to AYP and MSP in supported communities and schools.
• Able to assist with HIVSS self-screening as per the guidelines.
• Facilitate initiation on Prep for HIV-negative clients who are at risk of contracting HIV.
• Facilitate linkage to care for TB and STI's diagnostics and treatment.
• Provide Health talks on STI, TB, and HIV prevention and screening of HIV, TB and STI
• Ensure AYP and MSP tested for HIV are referred to other relevant services.
• Mobilize the AYP and MSP for PrEP uptake.
• Develop solid relationships with district DoH facilities, DSD facilities and other relevant
stakeholders to ensure effective linkages.
• Perform any other duties as assigned by the Professional Nurse, Safe Space Supervisor, and other
Manager.
• Develop and maintains a system of tracking newly diagnosed HIV, TB and STI clients telephonically
or home visits.
• Conduct follow up visits to facilities to ensure patient are captured into Tier.Net
• Monitor patient tracking logbooks and intervention to bring those lost to follow up into care
• Support quality improvement processes to strengthen linkage approaches and identify and
• Track and document patients who have transferred out of the facility.
• Participate in data analysis (facility targets, tracer indicators, cascades).
• Submit daily, weekly, and monthly data reports to the facility and the organisation.
• Complete HTS and Linkage to care register and other data collection tools on daily, weekly, and
monthly basis.
• Track linkage targets, prepare and submit monthly reports.
• Develop weekly and monthly plans accordingly.
• Participate in the facility/ organisation's weekly, bi-weekly, and monthly performance review
meetings.
• Understand and advocate for AYP's right to health care services.
• Be flexible to work on weekends.
• Perform any other responsibilities assigned by the Supervisor.
Minimum Qualifications
• HTS Counselling Certificate from an accredited provider.
• Adherence Counselling Certificate.
• Valid Driver's License and ability to drive.
• Experience in primary health care facilities and AYP in a community setting preferable.
• Experience of working with Key populations will be an added advantage.
Required Skills and Competencies
• Three years' experience in HIV and TB screening, HTS counselling, testing and linkage services and
• Understanding of the importance of the universal test and treat (UTT) policy, notably linkage to
care and retention of clients in care.
• Willing to travel and do work after normal working hours and on weekends.
• Ability to communicate in English, Setswana and/or another South African language.
• Ability to work with a multi-disciplinary team.
• Work experience in HIV and TB programme implementation.
• Good understanding of the South African public health national and provincial policies.
• Solid experience in and knowledge of partnering with multiple stakeholders.
• Extensive experience with client tracking (via telephone and in person through home visits).
• Previous work experience in NGOs or donor funded programmes will be an advantage.
• Computer literacy and competence in Microsoft software (MS Word, Excel and Powerpoint).
• Knowledge of HIV strategies, including the UNAIDS 95-95-95 targets, and TB management.
