They aspire to reduce social isolation and loneliness; increasing resilience and independence.They do this by assisting people to access support within the community, including support provided by a range of organisations, including the Voluntary and Community Sector.Work in the community and at various sites including:East London Foundation Trust (within Network of Primary Care)Aston-Mansfield Community Centre, Unit 1, 1 Toronto Avenue, E12 5JFCommunity Links, 105 Barking Rd, E16 4HQMind (Stratford Partners)About the ProgrammeThe Community Connectors programme is an exciting partnership between Aston-Mansfield, Community Links, Mind in Tower Hamlets and Newham, and the NHS East London Foundation Trust (ELFT).Community Connectors will work with ELFT (East London Foundation Trust ) under what is called the Mental Health Transformation Programme.The ethos of the programme is recovery-focused, exploring needs through complexity (rather than diagnosis), a focus on a person's strengths and assets, and on the wider determinants of health and wellbeing.Community Connectors support service users (18-65 years old) with social, practical, and non-clinical well-being issues.
They offer social interventions through a community-based approach.Community Connectors are not mental health clinicians, so are unable to provide medical guidance.
Instead, they work face-to-face in the community or over the phone, based on the needs of the service user.Community Connectors provide holistic support to patients suffering from a serious mental illness (SMI).
