Do you want to support individuals to help them improve their wellbeing by assisting them in accessing the right support within the community?
Community Connectors aim to empower individuals who are struggling with their wellbeing to make positive changes in their lives by supporting them in identifying their own personal goals. They aspire to reduce social isolation and loneliness; increasing resilience and independence.
They do this by assisting people to access support within the community, including support provided by a range of organisations, including the Voluntary and Community Sector.
Work in the community and at various sites including:East London Foundation Trust (within Network of Primary Care)Aston-Mansfield Community Centre, Unit 1, 1 Toronto Avenue, E12 5JFCommunity Links, 105 Barking Rd, E16 4HQMind (Stratford Partners)About the ProgrammeThe Community Connectors programme is an exciting partnership between Aston-Mansfield, Community Links, Mind in Tower Hamlets and Newham, and the NHS East London Foundation Trust (ELFT).
Community Connectors will work with ELFT (East London Foundation Trust ) under what is called the Mental Health Transformation Programme.
The ethos of the programme is recovery-focused, exploring needs through complexity (rather than diagnosis), a focus on a person's strengths and assets, and on the wider determinants of health and wellbeing.
Community Connectors support service users (18-65 years old) with social, practical, and non-clinical well-being issues. They offer social interventions through a community-based approach.
Community Connectors are not mental health clinicians, so are unable to provide medical guidance. Instead, they work face-to-face in the community or over the phone, based on the needs of the service user.
Community Connectors provide holistic support to patients suffering from a serious mental illness (SMI). Patients will have a single or range of diagnosed conditions, will be known to ELFT and will therefore have a range of needs, including symptoms of anxiety or depression, or have a diagnosis of schizophrenia.
Specifically, the Community Connector will work with multi-disciplinary teams (MDTs) who sit within primary care networks or PCNs (Primary Care Networks) (clusters of GP practices) across Newham and will support patients in accessing appropriate local services, embodying a true example of wrap-around support.
