Malmesbury Community Team are seeking a Therapy Clinical Lead with community experience who has the ability to clinically and operationally lead on the urgent response procedure for the team and the management of complex patient care in the community. You will be required to deputise for the Team Leader as well as work alongside a number of GP practices. The post-holder will provide a high level of clinical assessment, individualised care planning and expert advice. You will also be expected to contribute to national and locally agreed targets of reduction in unnecessary hospital admissions and the facilitation of earlier discharges from the surrounding hospitals.
You would be expected to work in patients' homes and community clinical settings in Malmesbury, Royal Wotton Bassett and Purton and Cricklade and will typically work core shifts.
The post requires you to undertake 3 funded modules at Masters Level; the post holder will be paid at Band 6 salary until they are completed and then will be moved to Band 7 salary.
Main duties of the jobExperience in front line NHS community services is essential as is evidence of up to date clinical professional development. Applicants need to have excellent clinical skills, well developed communication skills and be able to work under pressure. The role is demanding but rewarding, and we are looking for an enthusiastic, motivated and hardworking individual who can work flexibly and efficiently. A car driver and access to a car is essential for this post.
