Case Manager/ Social Worker

Our client is seeking a dedicated Case Manager/ Social Worker to coordinate and manage care plans for patients with chronic or complex medical conditions.
This role involves working closely with healthcare providers, insurance companies, and patients to ensure optimal care and efficient use of healthcare resources.
The Case Manager/ Social Worker will play a key role in patient advocacy, ensuring that patients receive necessary services while supporting the healthcare team in achieving clinical and financial outcomes.
The ideal candidate must be self-motivated, possess a strong work ethic, and demonstrate a "can-do" attitude.
They must also exemplify the company's Core Values—Teamwork, Resilience, Open-Mindedness, Ownership, and Positivity—while exercising discretion, diplomacy, and tact during patient and staff interactions.
Salary - $1500 Per Month Type of Work - Full Time (REMOTE) Work Schedule - US East Coast Time Zone Requirements In depth background in a remote set-up of work similar to freelancing as an Independent Contractor (deep understanding on the pros & cons involved in this industry).
Bachelor's degree in Nursing, Social Work, Healthcare Management, or a related field.
Master's degree preferred.
2-4 years of experience in case management, care coordination, or a similar healthcare role.
Case Management Certification (CCM) or eligibility for certification preferred.
RN license required for nursing professionals.
Strong communication and interpersonal skills, with the ability to convey information clearly to patients, families, and healthcare providers.
Ability to work effectively as part of a multidisciplinary healthcare team, ensuring collaboration between healthcare providers, external agencies, and insurance companies.
Proficient in using electronic medical records (EMR) and case management software.
Knowledge of healthcare systems, insurance processes, and medical terminology.
Excellent problem-solving and decision-making abilities, capable of handling complex patient cases and coordinating care efficiently.
Demonstrates flexibility in responding to changing patient needs, healthcare regulations, and case requirements.
With fast, reliable and stable internet connection (LAN or Fiber) and with internet backup.
