Job DescriptionThe Care Coordinator position at Hands On People (HOP) is integral to ensuring the effective delivery of services within our Home Care, Community Participation, and Transport programs. This role is focused on promoting the independence, empowerment, and well-being of participants through high-quality care and personalized support. The Care Coordinator is responsible for mentoring and providing direct supervision to staff, offering on-the-spot training, coaching, guidance, and support as needed.
In addition to managing a dedicated caseload based on participant needs, the Care Coordinator plays a key role in stakeholder engagement and collaboration within a multidisciplinary environment. Building strong relationships with participants, families, and care professionals is essential for achieving successful outcomes and ensuring participants receive tailored, responsive care.
Duties Schedule and organise engagement programs for our participants and communicate the activities with the team.Management of Community Access Participants files – Documentation, ensuring SA's are up to date, following up unsigned Service Agreements with participants and COS for community access.Assist in and coordinate, plan, and execute workflow activities including maintaining a professional relationship with participants, families, and other stakeholders.Ensure all participant reports meet compliance and reporting requirements of the NDIA and NDIS Quality and Safeguarding Commission and ensure case notes and other documentation of our participants are up-to-date and accurately compiled.Managing transportation services, engaging staff to cover these when required.Communication with the rostering team regarding shift change requests and transportation shift requests.Attending sign-ups/meet & greet for new Care Services participants.Conducting need-based assessments depending on NDIS goals by delivering quality services and interventions, as well as the reporting of tasks for auditing and review.Capacity to deliver high-quality documentation in line with administrative and compliance requirements.Coordinate medical appointment bookings, communications, family visits, activity plans, meal plans, and ensure seamless dissemination of information across team members.Workplace materials and documentation are maintained in line with legal obligations and best practice standards.Delivery of person-centered practice in daily operations.Travel to remote communities if required to fulfill the care needs of participants.Documentation associated with service delivery including case notes, medication records, and incident reports.Work with the Roster team in preparation of HOP Rosters.Ensure compliance with the on-call roster.Ensure attendance at the on-call meetings scheduled every Monday at 8:45 AM.Maintain records of on-call schedules, swaps, meeting minutes, and any changes made to the roster.Assist in other duties as required.Additional Position Responsibilities Managing and coordinating all site administrative processes and assets.Ensuring compliance with reporting requirements.Mentoring support workers to deliver high-quality support and achieve client and organizational objectives.Coaching and motivating team members, assisting with performance appraisals.Desired Skills and ExperienceRequired Education/Licenses/Certificates A B.Sc in Humanitarian Studies, Diploma or Cert IV in Disability or its equivalent.Current First Aid Certificate and CPR.OCHRE card/Blue Card.NDIS Worker Screening Check/ Yellow Card.Required Knowledge/Experience Minimum of 2 years experience in a similar capacity.Demonstrated skills in assessing, developing, implementing, and reviewing participant plans, goals, and supports.Attention to detail and strong administrative skills.Mentoring skills including experience in motivating, developing, and directing people in their roles while supporting productive and collaborative working relationships.Ability to develop and maintain effective high-level working relationships with all staff, government departments, health professionals, and other key stakeholders.
