Care Coordinator

Location: Remote (South Africa)
We seek an exceptionally warm, dedicated relationship builder to serve as our veterinary clinic's trusted ambassador for all cherished animals and their families.
As the Care Coordinator, your overarching purpose will be crafting personalized care experiences through compassionate outreach, mindful coordination support, and leading with education.
Core Responsibilities:

Conduct thoughtful outreach and follow-ups to forge enduring bonds with families
Develop trusted relationships with patients across our clinics
Guide conversations with empathy to explore patients' needs, preferences and lifestyle
Assess gaps proactively where additional coordination or preventative guidance could enrich pets' routines further
Identify opportunities to improve access, simplify navigation challenges, "fill in cracks" that may impede seamless journeys
Curate customized care packages - ranging from telehealth visits to specialists referrals to home health products - tailored to each pet's needs
Track patient progress and continuously identify opportunities to better support pets' quality of life
Report back insights to inform physicians and clinic leadership for improvement

We seek candidates with:

5+ years' in client services or healthcare coordination position (human or animal medicine)
Proven success developing deep 1:1 client connections and deftly navigating care transitions
Exceptional listening abilities to grasp unstated needs and personalize support
High EQ, warmth and nuanced communication style
Background in animal care, shelter management, veterinary support services, or pharmacy services
Systems thinker, able to visualize the full landscape of care access challenges
Self-directed organizer, comfortable managing complexity without pre-set protocols
Commitment to transparent relationship development vs transactional interactions
Sincere passion for animal wellness and family experience
Additionally desirable:
Licensed Veterinary Technician certification
Experience designing+implementing customer experience initiatives
Familiarity with telehealth platforms and virtual services coordination
