Hire Resolve is looking for an experienced and dedicated Case Manager to join a reputable healthcare facility in Mitchells Plain .
In this role, you will be responsible for coordinating and managing patient care services and ensuring that patients receive appropriate, high-quality care tailored to their individual needs.
Your expertise will be essential in developing care plans, collaborating with healthcare providers, and providing support to patients and their families.
Responsibilities: Conduct comprehensive assessments of patients' medical, physical, and psychosocial needs.
Develop, implement, and evaluate individualized care plans in collaboration with the healthcare team.
Coordinate patient care services across various settings including hospitals, outpatient services, and home care.
Serve as a liaison between patients, families, and healthcare providers to ensure effective communication and care continuity.
Monitor patient progress and adjust care plans as necessary based on evolving needs.
Provide education and resources to patients and families on health management and available services.
Maintain accurate and up-to-date patient documentation and case management records.
Advocate for patients to ensure they receive necessary services and benefits.
Participate in interdisciplinary team meetings to optimize patient care.
Stay current with changes in healthcare policies, procedures, and best practices.
Requirements Bachelor's degree in Nursing, Social Work, or a related healthcare field.
Minimum of 2 years of experience in case management or a similar role, preferably in a healthcare setting.
Strong understanding of healthcare systems and community resources.
Excellent communication, interpersonal, and organizational skills.
Ability to work independently and collaboratively as part of a healthcare team.
Strong critical thinking and problem-solving abilities.
Valid driver's license and willingness to travel as needed.
Certification in case management (CCM) is a plus.
Knowledge of electronic health records (EHR) and case management software.
