Purpose of the position:
To conduct follow-up and/or psychosocial activities in the PWID community by providing current accurate health information and linkage to care. Provide health care counselling, and guide for testing services in non-clinical sites linking screening of TB, STI, PrEP and SRH interventions and enhancing adherence by addressing the emotional and mental health matters. Promoting and encouraging enrolment into other HIV Prevention modalities (i.e. Social Network Strategies, Index, HIV Self-screening) implemented by the Programme.
Minimum Requirements
Grade 12 (level 4 qualification)
Counselling certificate, Finger prick certificate
Minimum 1-year work experience in the PWID industry or experience as a case manager
Knowledge of PrEP and ART guidelines
Drivers licence will be advantageous
Fluent in English and other South African languages
Computer literate in MS office
Ability to liaise at all levels and to work in a multi-disciplinary team
Proficient in report writing
Sound networks with related institutions
Knowledge of TB and HIV/AIDS
Duties and Responsibilities
Ensures fast-tracking referrals from teams in order to assess and provide direct psychosocial support to PWID
Provides appropriate interventions and/or referrals based on the needs arising
Assists clients with appropriate referrals for other issues related to psychosocial, (e.g. paralegal, rape, trafficking, substance abuse, etc.)
Provide an outcome report for referrals
Follow up with beneficiaries when required, particularly those lost to follow-up
Provides onsite and/or telephonic post-initiation and adherence counselling to link clients to treatment and retention in care
Conduct support/education group activities
Follow up clients who miss club sessions
Provides counselling and testing services as part of the outreach teams
Conducts and facilitates monthly adherence and other appropriate support groups to improve ART and PrEP retention
Prepare for Club sessions and refer clients to the club nurse
Assists in informally assessing service beneficiary response to services offered by the Programme during follow up visits by constantly checking in on their overall well being
Records and maintains accurate and detailed client's information and interventions and keeps data in order to submit as required
Ensures that all confidential records are kept safe according to the POPIA
Assist with managing crises and trauma, if needed
